What is the Discharge Medicine Service?

This video explains how to access support with taking your medicines from your Swettenham Chemists branch following discharge from hospital.

The service has been established to ensure better communication of changes to a patient’s medication when they leave hospital and to reduce incidences of avoidable harm caused by medicines. Learn more by watching this short video.
